All deductibles and charges are the responsibility of the patient and parent for insurance plans that are 80/20 or 70/30 type plan. We will be happy to give you an itemized statement for you to file your child’s insurance claims. We encourage you to ask our office manager for any fee information you may need.
Please get to know you child’s insurance plan, its benefits and non-covered items. Be aware of whether you have satisfied your yearly deductible for each child, if you have a deductible. Be prepared to pay your co-pay or co-insurance amount at the tine of service.
Please be aware that we do not file secondary insurance claims. We will be happy to provide you with an itemized statement for you to deal directly with your secondary insurance company.
Divorced / Separated Families
We know that coping with a divorce or separation can be difficult. However, we sincerely ask not to be a party to custody proceedings or billing disagreements. Therefore, the parent or guardian who brings the child to the office will be documented in the chart, and that parent or guardian will be responsible for the co-pay and any balance the day of the visit. We do provide records notarized copies of records for your custody proceedings at a cost of $30 per copy
Charity Health Care
Coppell pediatrics provides quality health care to a variety of patients. If your child is not covered by a Health Insurance Plan or Health Savings Plan, and you are unable to pay in full at the time of service, please notify the Office Manager or Billing Coordinator in advance of your visit.
Charity care billing does not include those who have chosen an Insurance Plan that does not cover specific services, such as well child exams, etc. Your choice of Health Insurance Plan determines your specific benefit coverage and your co-pay, co-insurance or deductible which is your responsibility to pay at the time of service.
We do require a signed statement regarding your financial inability to pay, and we do provide a discount to indigent patients whose parent or guardian is currently unemployed or disabled. We will establish a payment plan. However, payments not made within 45 days of each payment due, based on the signed payment plan, will be turned over to collections and to consumer credit reporting agencies.
