Medical Records

For the protection of our patients, our office does not release any medical information without the written authorization from the patient, parent or legal guardian, except when requested by law. The first copy of records you request for insurance or transfer to a new physician due to a move will be provided at no charge. Additional copies are $25, payable at the time of pickup. Notarized copies are an additional $5. Vaccine records for school or day-care are provided; however, please request 3-5 business days in advance.
